Description Job description: We are seeking a passionate and energetic RN - Memory Care Coordinator to lead our specialized programs for residents with Alzheimer’s disease, dementia, and other memory-related conditions. In this vital role, you will develop and implement engaging activities, coordinate care plans, and foster a supportive environment that enhances residents’ quality of life. Your expertise in senior care, recreational therapy, and working with individuals with disabilities will be instrumental in creating meaningful experiences for our residents. Responsibilities Design and facilitate engaging memory care activities tailored to residents’ cognitive abilities and interests Collaborate with nursing staff and caregivers to develop individualized care plans focusing on Alzheimer’s and dementia care needs Assist residents with Activities of Daily Living (ADLs), including personal hygiene, mobility, and medication reminders Coordinate with families and healthcare providers to ensure comprehensive support and communication Manage schedules for recreational sessions and community events to promote social engagement Provide counseling on aging issues and support for residents facing cognitive decline or behavioral challenges Maintain detailed documentation of resident progress, activity participation, and care interventions Experience Requires a current and unencumbered Proven experience in assisted living or nursing home environments specializing in memory care or senior care Strong background in recreational therapy, dementia care, or Alzheimer’s support programs Demonstrated skills in event planning and organizing community activities for seniors Familiarity with long-term care practices, home & community care settings, and caregiving responsibilities Excellent communication skills for engaging residents, families, and interdisciplinary team members effectively Experience working with individuals with disabilities or those requiring assistance with ADLs Knowledge of aging issues counseling, social work principles, and schedule management techniques Join us as a Memory Care Coordinator to make a meaningful difference in the lives of our residents by providing compassionate leadership, innovative programming, and dedicated support.
