Summary The Care Coordinator is instrumental in delivering supportive services to senior living communities and clinics. Assigned to a specific group of partner communities, the Care Coordinator develops meaningful relationships with residents, families, and staff, acting as a crucial link between the care team and the community. This role serves as a primary point of contact for patients with relevant conditions, collaborating closely with healthcare providers to optimize care, enhance patient outcomes, and improve overall quality of life. The Care Coordinator is essential in coordinating and implementing personalized care plans, providing ongoing support, and facilitating effective communication among patients, caregivers, providers, and other healthcare professionals. Key Responsibilities: Support dementia care initiatives through the Brigade dementia program Provide in-person assistance for telehealth provider visits Assist with care coordination, care plan updates, and patient enrollment Facilitate care coordination and communication of services for residents Offer companionship to community residents Participate in community and company events Provide event, marketing, and administrative support across community-based and company initiatives Undertake additional responsibilities as assigned Requirements: At least 18 years of age High school diploma, GED, or equivalent Medical Assistant Diploma REQUIRED Education or experience in patient care Education or experience in case management Possess outgoing, professional, and self-initiating qualities Ability to work independently Ability to stand for extended periods Willingness to travel to assigned locations daily Willingness to attend/work community events, which may occur outside the typical Monday to Friday, 8 AM to 5 PM schedule Phlebotomist License (Preferred) Join Us Today!
