Mental Health Case Manager – Kona $24–$27/hr | Full-Time | Mental Health Kokua If you like working directly with people and supporting recovery in the community, this is a solid, steady role with a team that appreciates it.   We’re looking for a Mental Health Case Manager who’s reliable, compassionate, and comfortable providing community-based support in Kona and West Hawaiʻi. What You’ll Take Care Of * Meeting with clients in the community to assess needs and provide ongoing support * Helping individuals create and work toward personal goals related to stability, health, and independence * Coordinating services and referrals with providers, hospitals, and community resources * Supporting individuals transitioning out of psychiatric hospitalization * Advocating for clients and helping them navigate systems and appointments * Completing required documentation and service records * Participating in team meetings, supervision, and case reviews * Adjusting to changing client needs while following program guideline Why People Like This Job * The work is meaningful and relationship-based * You get to spend time in the community, not behind a desk all day * The team is supportive and mission-driven * You can see the impact of your work directly in people’s lives * No two days are exactly the same What We’re Looking For * Around 1–2 years of experience in case management, behavioral health, or human services * Bachelor’s degree in a human services field preferred * Someone organized, dependable, and comfortable working independently * Strong communication and follow-through * Patience, empathy, and respect for people at different stages of recovery Requirements Standard pre-employment requirements apply, including background checks, TB clearance, CPR/First Aid certification, current driving documents and a valid Hawaiʻi driver’s license with the ability to travel throughout Hawaiʻi Island.
