We are seeking a full-time Mental Health Case Manager to join our community-based behavioral health team in Carlisle, PA. Schedule: Monday-Friday, 8:30 am-4:30 pm Earn $19.03/hour or $21.03/hour with the selection of Enhanced Pay, plus mileage reimbursement Position Details The Mental Health Case Manager plays a pivotal role in enhancing the mental health of mostly adults, children, and adolescents in Cumberland and Perry Counties. They coordinate resources and services, support families, and tailor care to each individual's unique situation. Their work focuses on harnessing individual strengths, connecting with community resources, and promoting stability. While operating independently, they are backed by a dedicated team, all within a dynamic community program. The case manager actively engages with consumers and their families in the home or community setting. Duties and Responsibilities * Coordinate and monitor all resources for adult and child consumers. * Assist consumers and parents in accessing mental health services. * Assess individuals’ strengths and needs. * Facilitate access to resources that enhance stability. * Provide assessment, service delivery, and problem resolution. Qualifications The ideal candidate will possess the following qualifications: Required: * Bachelor’s degree in the field of Sociology, Social Welfare, Psychology, Gerontology, Anthropology, Criminal Justice, Theology, Nursing, Counseling, Education, or other related social sciences; OR * High school diploma PLUS 12 college semester credit hours completed in Sociology, Social Welfare, Psychology, Gerontology, or other social sciences WITH two (2) years of experience providing direct services to mental health consumers; OR * Licensed Registered Nurse (RN). * Active and unrestricted driver's license with reliable transportation for work-related travel. Preferred Experience: * Prior experience working with children and adolescents. * Experience supporting individuals with serious mental illness strongly preferred.
