JOB SUMMARY:

This role is crucial in promoting optimum mental health among individuals and groups, addressing a range of issues from addictions and substance abuse to family dynamics, stress management, and self-esteem challenges. The ideal candidate must have excellent communication and documentation skills and possess an understanding of all mental health. The Caseworker will play an active part in the interdisciplinary team.

TRAINING REQUIREMENTS:

ALL PWP Health initial orientation training

Responsibilities

Participating in the person-centered planning process. Providing direct home-based behavioral health services and supports, including therapy for the family.
Assisting the team leader in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals.
Assisting the team leader with consulting identified non-medical providers, engaging community, and natural supports, and including their input in the person-centered planning process.
Meeting minimum billable units as assigned by the Program Director.
