Description Position: Mental Health Clinician II – Wellness Specialist, Flexible ACT Function: Serves as a clinical staff member with the lead responsibility for promoting health and wellness with clients and developing and implementing wellness strategies, conducting wellness assessment and providing education on healthy lifestyle choices. Reports to: Assistant Team Leader, Flexible ACT Location: The Bronx Schedule: Mondays through Fridays (9:00am-5:00pm) Tasks: Develop rapport with clients in order to engage them in improving their health and wellness. Administer standardized health and psychosocial risk screenings according to program protocols and timeframes. Utilize wellness screenings to identify interventions and to inform clients comprehensive care plan Work with clients to develop Wellness Recovery Action Plans (WRAP) to help them maintain their wellness and recovery. Collaborate with members of the client’s care team to identify needs and develop a plan to help client achieve optimal health outcomes. Implement tasks outlined on the care plan and ensure follow up and continuity of care between client interactions. Regularly reviews and update the care plan to correspond with services being provided. Document all interventions and attempted contacts in the EHR in accordance with program standards. Works with family members and other collaterals of the client’s choice to facilitate planning and delivery of care Facilitate crisis intervention in the event of an emergency. Facilitate care delivery by scheduling appointments, obtaining necessary information, and arranging transportation. Utilize evidenced based practices, such as seeking safety, motivational interviewing and cognitive behavioral techniques, to empower clients to grow and attain goals. Embrace the team model by collaborating with members of the team and providing support as needed Administer Client Service Dollar (CSD) funds and submits required documentation Participate in team meetings and weekly clinical conference Attend in-service training as requested Other duties as assigned by supervisor Requirements B.A. or M.A. degree in social services or a related field and one year of experience providing direct service in the human service field or nursing or Care Management/Service Coordination with demonstrated competency in clinical documentation, WRAP assessment, psycho-education, wellness self-management and cognitive behavioral strategies. Strong written and verbal communication skills.
