Employment Type: Permanent FTE: 0.80 FTE (32 hours per week)  Work Schedule: Weekdays - Dayshift Summary of Position Reporting to the Program Manager, Mental Health, the Mental Health Consultant collaborates with Primary care providers, Calgary Foothills PCN multidisciplinary teams and community/external partners to provide patient-centered care for those with chronic disease and/or complex health conditions. The Mental Health Consultant will support both our Extended Health Team, and our One-Step-at-a-Time Counselling Service (OSAT) – this is subject to change.  The Extended Health Team is comprised of health management nurses, clinical pharmacists, kinesiologist, occupational therapists, physical therapists, social workers, family physicians, nurse practitioners and mental health consultants. The team helps patients with complex chronic health conditions who struggle with psychosocial factors to self-manage chronic conditions, set achievable goals and access additional resources and support. Group and individual appointments or home visits are tailored to the patient’s needs. The OSAT program provides no fee, one-at-a-time counselling to people 18 years and older, couples or families.  This service provides support for people experiencing varying degrees of mental health challenges, emotional distress, difficult circumstances and intense interpersonal conflicts. It is patient focused and patient lead. Clinicians bring a broad spectrum of theoretical approaches; skills sets and knowledge to best assist the patient in their treatment journey. Key Responsibilities Key aspects of the position may include, but are not limited to:   * Contribute to the overall care plan of Extended Health Team patients by conducting a biopsychosocial mental health assessment, setting goals in collaboration with the patient directed at mental health concerns, providing a course of counselling including applicable mental health interventions to enhance patients’ coping and self-management skills, and contributing to regular progress review and case conference meetings, assuming the role of case manager as appropriate. * Provide one-on-one and group appointments * Work with patients on self-management by providing coaching and education to help advance their skills, knowledge and self-efficacy in adopting healthy lifestyle behaviours that are meaningful to them * Evaluate crisis situations and apply appropriate interventions * Provide single-session counselling appointments to individuals, couples- and families in the OSAT Counselling Service * Collaborate with other team members to help patients navigate the health care system * Help connect patients to applicable PCN, community and health system resources, including to specialized or longer-term mental health services * Promote continuity of patient care through effective written and verbal communication with the Patient’s Medical Home and those involved in the circle of care * Participate in the development, delivery, evaluation and quality improvement of PCN programs * Collect and maintain data for service indicators, progress reports and evaluations * Adhere to and perform activities and care within the full scope of practice as defined under the Health Professions Act Knowledge, Skills & Abilities * Preferred experience with the following therapy models: Cognitive Behavioural Therapy (CBT), Acceptance and Commitment Therapy (ACT), Dialectical Behavioural Therapy (DBT) Single Session model * Experience in dealing with health care issues, behavioural medicine and evidenced-based treatments for medical and mental health conditions an asset * Ability to work well in a team setting and independently * Experience with trauma informed care  * Group facilitation skills * Excellent communication (written and verbal), organization and critical thinking skills * Strong skills in collaborating with team members and physicians * Ability to prioritize, manage time effectively, and be flexible in a very dynamic work environment * Strong awareness of health care and community resources. * Good computer skills are required * Ability to work flexible hours, including evenings/weekends, when required * Accepts, implements and evaluates change with a positive attitude Qualifications * Minimum education of master’s degree and active registration in one of the following professions: * Social worker with authorization, or eligible experience for authorization, to perform restricted psychosocial interventions * Psychologist with clinical/counselling specialty or equivalent education/experience * Registered nurse with certification in Psychiatric and Mental Health Nursing OR relevant education and experience in providing mental health interventions * Occupational therapist with relevant education and experience in providing mental health interventions * Must possess a valid driver’s license and have access to a reliable vehicle Employment Requirements: * Completion of a satisfactory criminal record check and/or Vulnerable Sector Search.   Diversity & Inclusion * Our PCN strongly believes in sustaining an inclusive, respectful and equitable working environment that represents the communities we serve.
