Description This position is responsible for conducting mental health assessments, carrying a caseload, development of individual service plans, assisting in the development of a healthy mental state, and overall monitoring of residents’ involvement in implementation of a sober support network. Additionally, this position assesses, diagnoses, and treats mental illness and psychological distresses within the context of the family system. Duties and Responsibilities • Provides mental health screening, assessment, treatment planning, individual counseling, group treatment, and family intervention. • Based on program need, will be responsible for carrying a caseload. Facilitates counseling sessions and groups with residents, couples, and family members. • Works with residents to develop and reach treatment goals and revises goals as needed. • Provides residents with realistic crisis and/or safety plans as needed. • Conducts scales with residents to measure any increases or decreases in symptoms and adjusts interventions accordingly. • Coordinates with psychiatrists as needed. • Maintains up-to-date clinical documentation, progress notes, treatment plans, and discharge plans. • Corresponds with relevant family members and partner agencies. • Maintains knowledge of treatment modalities that are relevant to the resident population. • Maintains individual case records in compliance with agency policy and licensing authority regulations. • Coordinates services and maintains regular ongoing consultations with other organizations, managed care providers, legal agencies, etc. • Collaborates with other clinical and medical staff members in advocating for residents during Medicaid managed care and insurance company reviews. • Efficiently accesses other necessary services for residents as needed. • Utilizes person-centered care and evidenced-based practices. • Facilitates didactic/treatment phases and process groups. • Supports and encourages resident participation in community self-help groups. • Participates in supervision sessions with supervisor. • Facilitates admissions and orientations of new residents as needed. • Takes precautionary measures to limit transmission of communicable diseases (i.e., HIV, Hepatitis B, TB etc.). • Serves on agency committees as requested. • Carries out additional duties essential to the above-stated responsibilities as assigned. • Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners. • Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others. Requirements Possesses a relevant license such as a LMSW, LCSW, LMHC, or LMHC-P. Master’s Degree in Human Services or related field and a minimum of three years’ supervised counseling experience. Field placement as an intern may be applied to the required counseling experience. In addition to the qualifications above, a valid New York State driver’s license is required.
