Mental Health Practice Lead – Community Living Supports (CLS) Full-time, Permanent | Blacktown | SCHADS Award Level 4 | Service Delivery At New Horizons, the work we do changes lives and so do the people behind it. We're seeking a skilled and passionate Mental Health Practice Lead to join our Community Living Supports program in Blacktown; someone who shares our commitment to transforming lives and who is ready to bring their leadership, clinical experience, and values to this role. About the Role This is a key leadership position within our Community Living Supports program in Blacktown. As Practice Lead, you will guide and support a frontline team to deliver high-quality, recovery-oriented services to people living with mental health challenges, while also holding a small individual caseload of your own. You will bring both clinical credibility and genuine leadership presence to the role, helping your team to reflect, grow, and consistently deliver supports that are trauma-informed, person-centred and evidence-based. You will also work alongside Local Health Districts, housing providers, families and carers to build the integrated partnerships that make a real difference for the people we support. What You'll Be Doing Lead and model recovery-oriented, trauma-informed, person-centred practice across the team Practice Leadership Ensure the CLS model is embedded and consistent in day-to-day service delivery Maintain an individual client caseload alongside your leadership responsibilities Oversee the development and regular review of individual recovery plans Maintain timely, safe and responsive service delivery across the team Stakeholder Collaboration Build and maintain strong working relationships with LHD clinicians, housing providers, families and carers Contribute to connecting people to the right supports Contribute to reporting and evaluation against CLS KPIs Identify service gaps and drive quality improvement Ensure Compliance with relevant legislation, organisational policy and safeguarding requirements About You You are an experienced mental health professional who is equally comfortable leading a team as you are working directly with clients. You bring sound clinical judgement, a collaborative approach and a genuine commitment to recovery-oriented practice.
