We’re excited to welcome a Mental Health Professional to our Norfolk, VA team serving the Tidewater Region.   Position Details   As a member of the Assertive Community Treatment (ACT) Team, the Mental Health Professional plays a vital role in delivering comprehensive services to individuals with severe and persistent mental illness.  * Assess, plan, and develop individualized treatment plans for individuals with severe and persistent mental illness.  * Coordinate and provide therapy, medication management, and resource linkage services.  * Collaborate with a multidisciplinary team including psychiatrists, nurses, substance abuse professionals, employment specialists, housing specialists, and peer specialists.  * Conduct at least three home visits per week to deliver services in the individual's home or community setting.  * Utilize personal vehicles for client transportation and community-based work.  Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. Qualifications * Master’s degree in human services/related field * 3 years of clinical experience working with adults with severe mental illness.  * Knowledge and understanding of recovery principles and recovery-driven service system. * Driver’s License and willingness to utilize personal vehicle for work-related tasks.  Travel is necessary to offer support across the Tidewater Region.  * Licensed or Licensed eligible as LMHP or LMHP-R
