Job Description: We are working in partnership with Trinity Support & Care Services, who are an innovative organisation providing quality assured, safe, and person centered residential care and outreach support services to adults and children with intellectual disabilities. We are seeking compassionate and motivated Social Care Team Leaders to join their dedicated teams based in the Cookstown area. You will play a vital role in supporting people to live meaningful, independent lives; ensuring their dignity, rights, and individuality are respected at all times. This is a permanent contract, full time hours (days, nights, sleepovers) Monday-Sunday on shift rotation. Key Responsibilities: Provide person-centred, day-to-day support tailored to individual needs Promote independence, choice, dignity, privacy and human rights Assist with personal care and daily living activities respectfully Create and maintain a safe, clean and supportive environment Encourage social inclusion and community participation Support individuals to achieve personal goals and positive outcomes Work in a least-restrictive, enabling way using positive behaviour support approaches Provide clear information to support informed decision-making Accurately maintain care records, risk assessments and reports Administer medication safely and in line with policies and procedures Follow safeguarding, infection control and food hygiene standards Work collaboratively within a multidisciplinary team and contribute positively to the organisation Essential Requirements: Level 3 Health & Social Care qualification or equivalent Registered Social Care Worker with NISCC (or pending registration) Minimum 2 years’ experience in residential/social care settings Experience supporting people with learning disabilities, autism and/or mental health needs Experience of leading a team of staff in a social care environment, preferably residential care Competent in Microsoft Office and basic IT systems Full driving licence valid in the UK Legal right to work in the UK Desirable Requirements: Experience as a Key Worker or in a leadership role Experience supporting adults with complex needs or behaviours of concern Knowledge of RQIA Minimum Standards Training in Safety Intervention (formerly MAPA) or similar What we Offer: Comprehensive induction and ongoing training Career progression opportunities within a growing organisation A supportive and inclusive team environment Flexible working patterns to suit your lifestyle Opportunities to make a genuine impact on people’s lives every day To Apply Submit your CV via the APPLY button or give us a call on 02895313172 Stay in the loop with all our internal job postings by registering for our new Direct Medics Connect service!
