POSITION SUMMARY The Mental Health Social Practitioner works to support the community through social practice and help Members live their lives to the fullest, while developing independent living skills and making social connections. This position requires interest in community, creativity, and flexibility. In our Supported Housing Program, the Mental Health Social Practitioner will be based in a Fountain House Unit/s, 2 days a week for each unit. 1 day a week, they will be based in the Housing Resource Center with their Supported Housing Team. The Mental Health Social Practitioner will be responsible for their unit members in supported housing. During their unit-based days, the Housing social practitioner is expected to be working with their housing members. The Housing Social Practitioner will use their Housing Resource Center day to write their notes and service plans and for their Supported Housing Weekly Team meeting where the team will review member growth/obstacles and other issues that may arise. $30.58 per hour A day in the life of a Social Practitioner Requirements ESSENTIAL DUTIES AND RESPONSIBILITIES Responsible for a Resident Roster of approximately 30 members living in our stipend supported housing program Monthly face to face contacts with each member on their resident roster Quarterly Apartment visits Documentation: Monthly: Progress notes Service plans as needed Quarterly: Apartment Checklist Annually: Annual Stipend Recertification proof of income Grievance Policy review/signature Member Rights/Signature Surveys Proof of income Grievance Policy review/signature Member Rights review/Signature Surveys Limited Evening responsibilities Limited Transitional Employment Engagement with the members and social practitioners in their units REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES Detail-oriented and strong written, oral and communication skills Organized and punctual with deadlines General knowledge of AWARDS and excel preferred General knowledge of compliance regulations for supported housing preferred Ability to engage and develop relationship with members REQUIRED AND PREFERRED EDUCATION, EXPERIENCE, AND CREDENTIALS Undergraduate degree in related field required or related experience. At least 2 years of work experience in mental health programs and/or NYC housing programs Knowledge of AWARDS database highly preferred PHYSICAL REQUIREMENTS To perform this job successfully, an individual must be able to perform each essential duty and meet all physical requirements satisfactorily.
