HealthWISE is seeking a compassionate and motivated Support Coordinator to join our Ipswich-based wellbeing team. In this rewarding role, you’ll provide high-intensity support coordination for people living with severe and complex mental illness—helping them build resilience, access services, and work towards recovery in a safe and supported way.
This position is ideal for someone with lived experience of mental health recovery or who has supported someone through it, and who is passionate about making a difference in the lives of others.

• Deliver community-based, person-centred support
• Develop tailored Safety and Support Plans
• Work collaboratively with clinicians, GPs, and service providers
• Support clients to navigate the health system and improve social connectedness
• Provide outreach with some travel and overnight stays required
