Join our dynamic team providing essential community-based outreach and recovery-oriented support for individuals aged 16 and over.
As a Mental Health Support Worker, you’ll play a key role in developing and monitoring individualised recovery plans that empower consumers to achieve their personal goals. You’ll assist with assessments and deliver tailored support that promotes independence and wellbeing.
This role is part of the Housing and Accommodation Support Initiative (HASI) Plus, a statewide, high-intensity, transitional rehabilitation program. HASI Plus supports people living with severe mental illness and complex daily challenges, offering up to 24/7 clinical and psychosocial support in a recovery-focused framework.
About the role
The Mental Health Support Worker of HASI Plus will work with the HASI Plus, Clinical Lead, and Service Manager to ensure that the most appropriate and effective services are provided to consumers of the HASI Plus service.

Responsibilities
  • Assist in the delivery of individual assistance for consumers with a mental illness, based on individual recovery goals to support consumers.
  • Assist with the monitoring and review of consumer recovery plans, in partnership with Clinical Lead/carers/family members/clinicians/ community organisations as appropriate.
  • Promoting positive self-care strategies, enhancing social and daily living skills, increasing vocational capacity, and supporting community integration
  • Complete risk assessments and safety planning activities as required for consumers engaged with service
