POSITION SUMMARY

1 year contract - Maternity Leave
Cottage Country Family Health Team (CCFHT) is seeking a Masters-level Social Worker OR Registered Psychotherapist to join our interdisciplinary care team. The Mental Health Therapist will deliver high-quality, client-centered care to adults (18+) experiencing mild to moderate mental health issues.

QUALIFICATIONS

  • Masters-level Social Worker OR Registered Psychotherapist
  • Registered and in good standing with an appropriate regulatory college (e.g., OCSWSSW)
  • Minimum 3 years of experience delivering adult mental health services
  • Possession of professional liability insurance
  • Strong leadership, communication, and organizational skills

Responsibilities
  • Assess patients for eligibility and determine appropriate treatment plans
  • Obtain proper consent to collect and share information
  • Provide individual counseling and psychotherapy
  • Establish and coordinate personalized care plans
  • Facilitate interventions to support emotional and social well-being
  • Establish linkages to community and internal resources
  • Reassess clients to monitor progress and eligibility for continued service
