As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider. The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals as required. Human Resources Exemption: No Experience Experience with clinical interventions related to Primary Health Care Familiar with community resources and services Experience in brief treatment modalities in PHC setting is an asset Experience w/ clinical interventions in Integrated Care or Community Health Care Education Bachelor of Science in Human Services (BSHS) Licenses Valid Class 5 driver's license Competencies Excellent documentation skills Advanced - Communication skills Advanced - Critical thinking skills Advanced - Organizational skills Knowledge and Abilities Ability to work flexible hours Ability to evaluate the mind-body-behavior connection Knowledge/skills in the principle and practices that align to behavioral health Ability to follow structure and maintain routine Ability to treat patients with compassion and empathy Demonstrates ability to accept and implement change. Knowledge of Health Information Protection Act and other legislation. Ability to support improved functioning.
