As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider. The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals as required. Human Resources Exemption: No Experience Experience in brief treatment modalities in PHC setting is an asset Familiar with community resources and services Experience with clinical interventions related to Primary Health Care Experience w/ clinical interventions in Integrated Care or Community Health Care Education Bachelor of Science in Human Services (BSHS) Licenses Valid Class 5 driver's license Competencies Advanced - Communication skills Advanced - Organizational skills Excellent documentation skills Advanced - Critical thinking skills Knowledge and Abilities Knowledge of the Mental Health Services Act and Family Services Act Demonstrates ability to accept and implement change. Ability to evaluate the mind-body-behavior connection Knowledge of principles and implementation of behavioral health skills Adherence to the Code of Ethics. Knowledge of Health Information Protection Act and other legislation. Ability to work under stress Ability to support improved functioning.
