How you will make a difference (job overview): This position is responsible for conducting initial assessments and developing and updating treatment plans; leading admission and discharges planning process and providing therapeutic services. Division/Program Overview: 24/7 voluntary residential setting specifically for Transitional Age Youth (TAY) at risk and needs a higher level of care, but lower than a psychiatric hospital. Facility equipped to serve up to 15 Youth. Provides mental health services that include individual therapy, group rehabilitation, medication management, and case management. Incorporates the tenets of recovery, peer support, and identifying community connections. Connects individuals to educational and vocational services and initiating the process of locating housing.
