Description Our Opportunity: Under the general supervision of the Mental Health Clinical Supervisor the Mental Health Therapist offers assessment, diagnosis, and therapeutic interventions to meet client driven goals which assist with managing mental health symptoms and psychosocial issues. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate stabilization and maintaining the most optimum living conditions for the client in the least restrictive setting. Provides clinically based and empirically supported therapeutic interventions and care coordination to facilitate the stabilization and management of mentally ill clients and assists in the identification of appropriate utilization of resources across the continuum of care. Unmet needs of clients and the community will be evaluated and responded appropriately by creating group opportunities, psycho-education, community outreach and other duties as needed. The therapist works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the overall management of clients care in the community mental health setting. Delivers direct service to clients in the form of individual, family, and group therapy. Works with a variety of ages and presenting problems. Our Mission: We exist because people matter. Who are we? Tri-Cities Community Health has grown into a comprehensive non-profit Federally Qualified Health Center (FQHC) where we have evolved into a network of community health centers that continue with the foundation TCCH was built on and serve an even more diverse population in the Tri-Cities and surrounding areas. Essential Functions: Directs, coordinates, and provides mental health therapy and coordination to clients in caseload Interacts, communicates, and intervenes with multidisciplinary team in a purposeful, goal-directed fashion. Works proactively to maximize the effectiveness of resource utilization and client service delivery Maintains current knowledge of therapeutic modalities, therapeutic interventions, and agency guidelines for treatment, as specified by federal, state, and private insurance guidelines Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job related hazards Maintains Documentation of client care Implements interventions that lead to the client accomplishing goals established in treatment plan. Coordinates the necessary resources to accomplish goals developed in treatment plan. Gathers information from sources to enable the mental health therapist to monitor the treatment plan’s effectiveness Demonstrate an ability to collect essential information and appropriately interpret such information in order to activate necessary resources Be familiar with community resources for appropriate referrals. Coordinate services with identified individuals on Treatment Plans who have a signed Release of Information. This may include family members, friends, school administrators, previous counselors, and primary care providers. Ensures clients have access to crisis services via 24 hour on call services. When hospitalized in an inpatient setting the clinician will coordinate with discharge planning so the client can successfully transition back to an outpatient provider Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment Promotes effective working relations and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives Ability to carry out all other duties as assigned **This is a bargaining unit position** Schedule: Monday through Friday, 7:30 AM - 4:30 PM Salary: $35.81-$40.33/hour; up to $46.32/hour Requirements Minimum Education: Master of Social Work required. Completion of a Council on Social Work Education (CSWE) accredited MSW program required. Minimum Work Experience: One year of experience in a mental health setting preferred. Required Licenses/Certifications: Licensed Independent Clinical Social Worker or Licensed Independent Clinical Social Worker Associate license through the Washington State Department of Health required. Current Driver’s license and auto insurance required. CPR/First Aid preferred.
