Job Description Migrant Outreach Coordinator / Medical Assistant The Migrant Outreach Coordinator will perform medical assistant activities such as taking vitals, vaccination or drawing blood. Establish and maintain an effective and efficient record of lab procedures on the Mobile unit. The Migrant Outreach Coordinator approaches his/her tasks in a team based care fashion that supports patients and their families in self-management, self-efficacy and behavior change. When needed will be responsible for entering the information of the patients served on Mobile Unit into Salud’s system, including verification of third payer status and generation of claims for services offered in Mobile Unit. Will do outreach activities to promote the health of the different communities in SALUD service area when needed. Adheres to the spirit of the SALUD mission statement while performing assigned duties. We offer: $20.50 to $27.00 per hr. Monday to Thursday – 4-10 hour days per week In the Winter, could start as early as 5 am, and in the Summer, could work as late as 10 pm. Flexible Spending Accounts Basic Life and AD&D Supplemental Life and AD&D Short-Term & Long-Term Disability Accident & Critical Illness Insurance Employee Assistance Program Our Mission is to provide a quality, integrated healthcare home to the communities we serve Specific Duties: Perform Medical Screenings such as blood pressure, random blood sugar and hemoglobin as well as vaccination and blood drawing to patients served in Mobile Unit Works in conjunction with other Medical Providers, Dental Providers, Behavior Health Providers and all other staff to ensure that the patients obtain the highest standards of integrated healthcare The Migrant Health Coordinator acts as a patient advocate for our vulnerable population by ensuring that the patient is fully informed and comfortable with their visit to SALUD Ensure that patients are fully informed and comfortable with their visit to Salud Mobile Unit Collects accurate and updated information at each patient visit Be an active resource to help and facilitate referrals to Salud Clinics to patients seen within Salud’s Migrant Program When necessary assists with translation/interpretation for patients Establish and maintain an effective and efficient record of lab procedures on the Mobile unit Ensures that the lab equipment is working properly Provides lab reports to Migrant Health Director Navigate patients in the registration process when obtaining services in Mobile Unit as well as serving as liaison between Mobile Unit and Salud Clinics when needed. Act as back up for IZ coordinator Follows procedures for the electronic health records system Perform other duties as assigned by the Migrant Health Director Qualifications Education: High school diploma or GED Experience: Bilingual in English and Spanish required Licensure/Certification: Medical Assistant Certification Knowledge, Skills and Abilities: Ability to develop and maintain an effective working relationship with other Salud employees, the different health departments and other human service providers Must work well with the other patient care team members Knowledge of principles and processes for providing customer and personal services.
