Position Summary: We are seeking a dedicated and compassionate Service Coordinator to join our team. The Service Coordinator will play a critical role in guiding families through the early intervention process. We are seeking a highly organized, detail oriented, compassionate individual to fill a full-time position in the Clay, Platte and Ray counties in Missouri. Candidates must be able to travel regularly to locations within their assigned region and to the satellite office in St. Joseph, Missouri as needed.

BACHELOR’S DEGREE OR HIGHER IN ONE OF THE FOLLOWING WITH ONE YEAR OF DOCUMENTED EXPERIENCE WORKING WITH FAMILIES AND YOUNG CHILDREN:

Responsibilities
  • Serve as the primary point of contact for families enrolled in the Missouri First Steps program.
  • Conduct family assessments and gather information on the child’s development.
  • Develop, implement, and monitor Individualized Family Service Plans (IFSP) in collaboration with families and other professionals.
  • Coordinate services such as ongoing therapy, initial evaluations, and assessments with families and providers.
  • Facilitate monthly Early Intervention team meetings.
  • Provide families with information and resources to promote their child’s development and family support.
  • Ensure that services are delivered according to state and federal regulations.
  • Maintain accurate and timely documentation of all services and communications.
  • Coordinate with other medical and healthcare providers.
  • Facilitate the transition from the First Steps Program.
  • Participate in ongoing training and professional development.
