JOB SUMMARY

To reduce instances of re-hospitalization among peers being discharged from inpatient settings by providing transitional mental health peer support and increased engagement with community based services and natural supports.

HIGH SCHOOL DIPLOMA OR GENERAL EDUCATION DEGREE (GED)

Completion of a peer advocacy training program or related certification is preferred. Academy of Peer Services Certification must be completed within the employees first (6) Six months in this position if the employee is not currently certified.

Responsibilities

ESSENTIAL FUNCTIONS:

  • With the mobile crisis team: shall attend hospital discharge planning meetings to assist in coordinating support for the Clients.
  • Assist clients in transitioning from inpatient to community based settings via provision of peer support interventions, crisis intervention and community integration.
  • Work in conjunction with a clinical team to provide crisis intervention services during non-traditional work hours.
  • Provide peer support to help Clients adjust to their issues and community.
  • Refer Clients to other governmental and non-governmental agencies.
  • Coordinate various activity and support programs with other social agencies.
  • Compile and assist with record keeping.
  • Collect and maintain appropriate data from various sources such as professional staff and Client’s families to establish an overall approach to the solution of Client’s issues.
  • Complete documentation in a timely manner as outlined in the DCS Policy and Procedure and in keeping in funder requirements.
  • Assist in maintaining and reporting statistical data necessary for program evaluation.
  • Maintain information on community and health resources which can be utilized during the Client’s care.
  • Participate in the process of transferring Clients to other levels of treatment as appropriate.
  • Be responsible to comply with all funding source regulations, for creating a safe environment, and to report any unsafe situations to their immediate supervisor.
  • Must be enthusiastic about promoting the mission of the CCA and Housing Options; willing to work towards the good of the entire agency, and to act as an ambassador throughout the community.
