Key Responsibilities Provide social work services under a physician’s plan of care, supporting patients’ emotional, social, and environmental needs. Assess psychosocial factors affecting care and help families access financial, community, and supportive resources. Partner closely with the interdisciplinary team to develop individualized care plans and ensure smooth care transitions. Support patients and families as they navigate complex family dynamics, offering clarity and steadiness during difficult conversations. Maintain timely documentation, participate in care conferences, and uphold all Medicare, state, and professional standards. Requirements What We’re Looking For Master’s in Social Work (MSW) from a CSWE-accredited program; we welcome newer graduates with strong clinical rotations. Empathetic communicators who bring calm, clarity, and confidence to emotionally complex situations. Individuals skilled in navigating difficult conversations and building trust with patients and families. Self-directed, reliable professionals who thrive in field-based work and value collaboration with clinical partners.
