POSITION SUMMARY The Medication Therapy Management (MTM) Pharmacist will be responsible for completing Comprehensive Medication Reviews (CMR), assisting in enhancing the MTM program, and performing day-to-day operations of the University of Hawaii at Hilo (UH Hilo) MTM Center.  The MTM Pharmacist will also be expected to guide, train, and supervise applicable staff and pharmacy students on program operating policies/procedures, etc. Locations: On-Site Hilo and and remote opportunities available (must reside in Hawaii) ESSENTIAL JOB DUTIES AND FUNCTIONS * Medication Therapy Management.  Coordinates the execution of the program from scheduling, conducting, and following up on outreach. Delivers MTM services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews and Targeted Interventions to assess appropriateness of all medications, identify barriers to adherence, and optimize medication therapy to improve clinical outcomes. * Provider Collaboration: Communicate directly with physicians, healthcare prescribers and staff about the MTM program and to recommend therapy adjustments, alternative medications, or cost-saving opportunities. * Supervision.  Maintain the operational integrity of the program from mentoring, coaching, and educating the MTM staff along with pharmacy students (i.e. student interns). Supervision includes discussion prior to student CMR outreach and review/confirmation of CMR completion and outgoing materials.  * Compliance.  Ensures compliance with requirements of all third parties (e.g. HMSA) contracts, University of Hawaii at Hilo and University Health Partners, Inc. (UHP). * Communication.  Responsible for communicating with all contracted third parties/insurers and their members that are eligible for services, applicable pharmacies, and providers.  * Meeting Attendance.  Responsible for attending staff and agency meetings and reporting pertinent information obtained to pharmacists/staff. * Scheduling.  Responsible for ensuring adequate staff to adhere to all scheduled appointments. * Liaison.  Acts as a liaison between the UH at Hilo, UHP and third party (HMSA) staff regarding various departmental issues. * Troubleshooting.  Vendor oversight of the operational end user interface and ideas for enhancement of the system. * As Needed.  Identification and reporting of clinically significant medication concerns to appropriate provider staff. Assist with performing research, data collection, and reporting duties for UH Hilo to HMSA.   * Complies with all legal requirements and company policies. * Performs all other duties as assigned. QUALIFICATION REQUIREMENTS Education/Training/Experience Minimum: * PharmD or Bachelor of Pharmacy equivalent * Licensed Pharmacist by the State of Hawaii Board of Pharmacy * 24 months experience as a practicing pharmacist in a community/retail, ambulatory care setting or accredited residency equivalent. * Demonstrated experience conducting Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs). Preferred: * MTM Certfication Skills Knowledge Minimum:  * Strong communication skills (both oral and written). * Good organizational and oversight skills. * Proficiency with MTM platforms (e.g.,Arine) and Electronic Health Records (EHR). * Knowledge of pharmacy/medicine and common operating procedures/practices. * Strong technical (medical) writing ability. * Knowledge/Experience, spreadsheets, shared drives, etc. COMPENSATION & BENEFITS   * Pay rate: $60/hour * Retirement plan with 3% dollar-for-dollar company matching contributions   * Employee assistance program (EAP)   * Pet insurance with competitive premium rate    UHP is an EEO/AAP Employer.
