About the Role: Join Patient Navigation Services as our Team Leader! Lead a vibrant multidisciplinary team supporting adults and children with complex health and psychosocial needs. Our services (LaNS and PLaNS) help consumers and families navigate the health system, ensuring they get the right care, at the right time, in the right place. As Team Leader, you’ll provide clinical guidance, project support, and service leadership across both adult and paediatric pathways. You’ll drive integrated, patient‑centred, trauma‑informed care and strengthen connections across primary, acute, and community services. Make a meaningful impact by transforming how people experience their care journey. This is a multi-classified position with possibility of appointment at RN3.1, HP4 or P4.
