About our Unit: The Moir Centre for Complex Continuing Care (CCC), is a 39-bed unit that provides care for patients who are medically stable but have complex health needs that require specialized or technology-based care.    At the time of admission to CCC patients must have care needs that cannot be met in the community or in a long-term care facility (examples include tracheostomies, PICC lines, and complex wound care). The goal of CCC is maintaining or slowing the further loss of function, and the CCC team works closely with patients and their caregivers to promote wellness, improve quality of life, and help achieve goals of care set collaboratively with the patients and their caregivers. The CCC team is interprofessional, and offers maintenance therapy to patients in order to optimize their overall health and restore their maximum level of independence, with a goal of transition to other settings whenever possible.   Position Summary:  Reporting to the Manager, Complex Continuing Care, the successful candidate will conduct client assessments, develop treatment plans, implement therapy processes, and evaluate progress for patients within the Complex Continuing Care program.  Active collaboration with the interprofessional team is an integral component of this position. Music therapy is a discipline in which Certified Music Therapists (MTAs) use music purposefully within therapeutic relationships to support development, health, and well-being. Music therapists use music safely and ethically to address human needs within cognitive, communicative, emotional, musical, physical, social, and spiritual domains.  The services performed by a certified music therapist (MTA) include the application of music therapy interventions within the context of a therapeutic relationship. This relationship is developed through music-based, verbal, and/or non-verbal communication. Certified MTAs use music to address human needs within cognitive, communicative, emotional, musical, physical, social, and spiritual domains. This scope of practice includes a range of procedures, actions, processes, roles, and responsibilities that a certified MTA can safely and effectively perform (Canadian Association for Music Therapy).   Key Responsibilities:  Patient Care: * Participate, as part of the interprofessional team, in goal setting, evaluation and discharge planning * Use appropriate assessment skills to help establish a plan of action by prioritizing and setting goals of treatment in conjunction with patient * Provide education, and make recommendations to appropriate community programs/services in conjunction with patient to set an effective discharge plan * Obtain and document informed consent for assessment and treatment by a Music Therapist * Communicate patient assessment findings, treatment program, and progress with other health care professionals involved in the patient’s care and share responsibility for development and implementation of interprofessional care where indicated * Participate in weekly interprofessional progress rounds, monthly professional practice meetings, business meetings and program meetings, as schedule allows * Participate in the co-ordination of patient treatment and communicate with other community health care professionals within the “circle of care” * Assist and participate in developing, implementing and practicing quality improvement within the program/unit * Liaises with external care providers to ensure care needs are met and safety is maintained * Maintain and research community partnerships with community programs/services for effective discharge planning    Non-Patient Care: * Document relevant assessment data, keep current and timely progress and discharge notes for all referred patients while following the professional standards * Document required workload measurements data in a timely and accurate manner * Assist in the education, supervision and orientation of Music Therapy interns, students and volunteers in the hospital  * Maintain a focus on continuing professional development by attending and presenting in-house sessions and attending continuing education courses out of hospital and taking part in ongoing reflective practice   Qualifications:  * Member in good standing with the Canadian Association for Music Therapy (CAMT) * Successful completion of a minimum bachelor of music therapy degree at a CAMT recognized institution, although a Master’s of Music Therapy is preferred * Completion of a 1000-hour clinical internship under the supervision of an accredited Music Therapist (MTA) * Current registration (or qualifying member) with the College of Registered Psychotherapists of Ontario preferred * Gentle Persuasive Approach (GPA) certification preferred * Experience in provision of Music Therapy programs for adults  * Demonstrated leadership skills and evidence of continuing education required * Experience in program development, including adapting therapeutic modalities to support medically complex patients, including individuals with cognitive and physical disabilities, and mental health needs * Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high performance interprofessional team     To pursue this career opportunity, please visit our website: www.trilliumhealthpartners.ca [http://www.trilliumhealthpartners.ca/]  Trillium Health Partners’ (THP) is an equal opportunity employer who values the importance of antiracism work and is committed to integrating antiracism, diversity, equity and inclusion best practices throughout THP operations, policies and culture.
