About the role The Navigator plays a crucial role in facilitating access to important appointments and services for Clients. Working closely with the case manager, the Navigator provides transportation and logistical support, ensuring timely and safe travel to appointments and/or other services. This position serves the SDOH needs of the client, while fostering positive relationships and promoting a supportive environment. By addressing transportation barriers and coordinating care, the Navigator contributes to improved outcomes and enhances the overall quality or care provided. What you'll do Review appointments for following shift, communicating with medical, case management and the Client. Gather, print and review all necessary or related documents for upcoming appointments Transportation of patient(s) to necessary appointments, government agencies, and medical or behavioral health facilities Maintaining the schedule for reoccurring group appointments Maintain community connections for efficient coordination of care Communication and cooperation with police, probation officers, governmental agencies, nonprofit organizations, and other organizations as needed Serve as a guide for the client and apply knowledge related to navigating the criminal justice system Maintain communication with the care manager(s) and care coordination team for reliable transportation and task assignment. An expectation of the role is to check in with the team upon arrival to and prior to departure from the clinic. Facilitate groups as needed Document all Client interactions in the EMR in an accurate and timely manner Other related duties as assigned by the leadership team. Lifting up to 50 pounds may be required infrequently This job may require being outside in extreme weather for limited periods of time This job may have exposure to bodily fluids: PPE is provided by the company. This type of work involves sitting, standing, walking, bending, and stooping from brief periods of time to lengthy periods of time. This position may require ascension and descension of one or more flights of stairs throughout the shift. This job will require driving at various times of the day with multiple people in the vehicle. This job may require personnel to work weekends or nights. This job may have the opportunity for overtime shifts. To perform this job successfully, an individual must be able to perform each essential job duty satisfactorily. Qualifications Minimum of high school diploma or GED Minimum of 6 months of experience in working in the field of substance abuse treatment or behavioral health-related field CPR/FA Certification. Must be at least 21 years of age Driver's License with no restrictions Driving responsibilities include transporting clients to and from treatment appointments. For insurance compliance and safety standards, candidates with a history requiring SR-22 insurance will not be considered for driving roles.
