Position Summary
Under the supervision of the Assistant Program Manager, optimizes the home environment for the physical, mental, and emotional well-being of children who are at risk of being abused or neglected, assists in strengthening the parent-child relationship by improving
parenting skills, reducing family stress, and meeting needs of parents. Aids in the prevention of child abuse and neglect in at-risk families by providing direct services to families, providing collaborative and consultation services to the Family Support Specialist (FSS),
and providing community outreach. Service provision in this position is primarily field based in the clients’ homes.
Essential Functions
Initiate voluntary care coordination services to Healthy Families Florida (HFF) participants as it relates to substance
misuse, domestic violence, and mental health.
Manage and coordinate referrals and initiate staffing of cases with Supervisor or Assistant Program Manager.
Conduct initial staff consultations for referred families to determine strengths, extent of any problems, the families’
readiness to engage in treatment and barriers to intervention.
Conduct home visits with referred participants to determine the referrals needed, barriers to accepting services and
additional needs of the family, with or without the Family Support Specialist.
Identify suitable care providers and connect participants with the appropriate level of care, based on their clinical need
and community provider availability.
Follow-up with family and service providers on specialized services as needed.
Develop a referral plan with input from families, FSSs and Supervisors to address concerns identified at the initial assessment or during services.
Work with the family in partnership with the FSS and accompany FSS on home visits, as appropriate.
Qualifications:
EDUCATION/TRAINING
Bachelor’s Degree in Psychology, Social Work, Counseling, Nursing, or similar degree in the social service field. Pay range is $22 to $27 an hour based on experience.
MINIMUM OF TWO (2) YEARS’ EXPERIENCE PROVIDING REFERRAL AND CASE MANAGEMENT TO THOSE AFFECTED BY MENTAL HEALTH ISSUES, SUBSTANCE ABUSE AND DOMESTIC VIOLENCE. EXPERIENCE CONDUCTING HOME VISITS WITH HIGH-RISK FAMILIES PREFERRED.
Experience can have been gained through previous employment or related activities. Must demonstrate a knowledge of and belief in non-violent approach to disciplining children. Must demonstrate knowledge of child development, family dynamics, family problems, and
stresses of parenting Experience in working with or providing services to children and families, acceptance of individual differences, experience and willingness to work with the culturally diverse populations present among the site’s target population, open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision, etc.
