The Opportunity The Navigator I plays a critical role within Eon’s Care Management program. This position engages patients and providers at pivotal moments when timely follow-up matters most. Using the EPM platform, Nav 1s support patient follow-up by communicating with patients and provider offices, coordinating appointment logistics, and tracking adherence to care pathways. While this is a non-clinical, administrative role, it sits at the heart of our mission: helping patients move through a complex and often frustrating healthcare system so they don’t fall through the cracks. As a Navigator I you will: Conduct structured outbound calls to patients and providers to coordinate overdue or upcoming follow-up care. Handle inbound calls from patients and provider offices, using approved protocols to respond with empathy and professionalism. Help patients overcome logistical barriers to care (scheduling confusion, understanding next steps, connecting with offices). Follow standardized call scripts and workflows to ensure quality, compliance, and patient/provider satisfaction. Accurately document all interactions, call outcomes, and update patient records within the EPM platform. Identify and escalate discrepancies, unclear findings, or complex inquiries to the appropriate healthcare provider or supervisor. Collaborate with internal teams to uphold data accuracy, system integrity, and efficiency. Support internal process improvement and participate in quality assurance initiatives. You will be a good fit if you can: Keep the patient at the center of every interaction, bringing empathy, patience, and follow-through. Communicate clearly and compassionately across a wide range of patient and provider personalities. Balance administrative accuracy with a patient-centered mindset. Maintain composure in a fast-paced, hybrid work environment. Follow standardized workflows while knowing when to escalate concerns. Remain self-motivated and accountable with minimal supervision. Qualifications and Skills: Associate degree or higher in a healthcare-related field is preferred Healthcare experience required (minimum 2 years in a medical setting preferred). Foundational knowledge of medical terminology (required). Proficiency with Apple MacBook and Google Suite (preferred). Excellent verbal and written communication skills. Ability to work in a fast-paced, high-volume environment. Highly organized with the ability to manage multiple tasks and meet deadlines.
