Description

Responsible for planning, coordinating and evaluating the navigator service provided to individuals accessing the IBPS across the region. Assess and evaluate beneficiaries emotional, social, practical, and financial needs in collaboration with the specialist multi-disciplinary team, and support or signpost accordingly E.g., advising on housing, finances, and benefits problems at work and at school. Respond independently to crises in conjunction with medical staff, IBPS psychological professionals, and the multi-disciplinary team. Liaise and provide specialist knowledge on the social needs of the Infected Blood community to both statutory and non-statutory agencies within the hospital and the community.
Maintain regular data collection and analysis documenting the important supportive and preventive work that the specialist navigator provides. This will be facilitated by a national case management system. Maintain accurate and up to date documentation of all interventions with individuals, taking account of their rights to access records under the Freedom of Information Act
