Quest Collective is a Brisbane and Sunshine Coast NDIS provider delivering Supported Independent Living, ILO informed supports and one-to-one mentoring for teens and adults. We're not trying to be the biggest provider on the Coast. We're building the most trusted, person-centred, values-driven one. The role

We call our support workers mentors, because that's what the job actually is. You won't just be filling shifts. You'll be walking alongside participants as they build real capacity and independence, whether that's in one of our SIL homes, an ILO shared home, or through one-to-one mentoring in the community (minimum 15 hours per week per participant).

Every shift follows a clear structure, every participant has real goals, and you'll be part of a team that raises the standard rather than coasting. We believe the participant is the expert on themselves, and our job is to back them.

What you'll need

  • 2+ years experience in NDIS, child safety or community services
  • Experience supporting people with complex behaviours
  • Blue Card, Yellow Card and NDIS Worker Screening (or willingness to obtain)
  • Cert III Individual Support (Disability), held or willing to obtain
  • Full open driver's licence and right to work in Australia

The NDIS Care Coordinator is an entry-level role designed for individuals looking to build a career in the NDIS sector. You'll work closely with experienced NDIS Support Coordinators to learn how to assist participants in navigating their NDIS plans, connecting with services, achieving their goals and building their capacity. This is a great opportunity to develop foundational knowledge of the NDIS while making a meaningful impact on the lives of our community members. Full training and mentorship will be provided to help you grow into a Support Coordinator role.


Duties


  • Support participants in understanding the basics of their NDIS plans and available supports.
  • Assist in linking participants with appropriate service providers, community resources, and culturally safe supports.
  • Learn to develop simple action plans in collaboration with Support Coordinator, focusing on short-term goals.
  • Provide administrative support, including maintaining case notes and ensuring compliances with policies.
  • Observe and participate in meetings with participants, families, and stakeholders to gain hands-on experience.
  • Develop a strong understanding of culturally safe and person-centred approaches to service delivery.
  • Participate in training and mentoring opportunities to build skills for future career progression.

Skills and Experiences:

Required


  • Certificate IV or higher qualifications in Health Care, Social Assistance, Community Services, Administration and/or one year disability/age care experience.
  • Demonstrated ability to record accurate and consistent case notes.
  • Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
  • Strong organisational skills and ability to work independently and/or as part of a team.
  • Ability to communicate with sensitivity and effectively with Aboriginal and Torres Strait Islander people.
  • Sound knowledge of Microsoft Suite Applications.
  • Current Drivers Licence (QLD).
  • Current National Police Clearance or ability to obtain.
  • Working with Children's Crad (Blue Card) or be eligible to apply.
  • Disability Worker Screening (Yellow Card) or be eligible to apply.
  • Willing to travel to our other locations.

Desirable


  • Previous experience of working with Aboriginal & Torres Strait Islander clients.
  • Previous experience in working with the National Disability Insurance Scheme (NDIS).
  • Previous experience in case management and coordination activities using a clients information system.
  • Demonstrated ability to meet targets and performance outcomes.
