About the Role
As a NDIS Case Manager, you will be responsible for managing end-to-end support coordination and case management services for participants under the NDIS. You will be at the heart of the care journey — developing individualised care plans, coordinating services, conducting risk assessments, and ensuring quality, compliance, and participant empowerment.

Key Responsibilities

  • Comprehensive Case Management: Oversee and coordinate holistic support plans tailored to each participant’s NDIS goals and funding.
  • Intake & Onboarding: Lead initial engagement and onboarding, including needs assessments, risk identification, and service suitability evaluations.
  • Care Planning & Roster Management: Design person-centred care plans and service schedules in collaboration with clients and their support teams also review Shift notes daily on both ShiftCare and Sameview platforms.
  • Assessments & Progress Reviews: Conduct regular functional assessments, track progress, and revise support strategies as required.
  • Team & Stakeholder Collaboration: Engage regularly with participants, families, support workers, allied health professionals, and other stakeholders to deliver cohesive care.
  • Documentation & Compliance: Ensure case notes, incident reports, care plans, and risk assessments are accurately completed and meet all NDIS and organisational requirements.
  • Risk & Crisis Management: Act promptly in crisis situations, managing participant risks while maintaining safety and wellbeing.
  • Staff Guidance & Leadership: Mentor SIL managers and support workers, address complex issues, facilitate team meetings, and support staff performance reviews.
  • NDIS Compliance & Reporting: Prepare reports for SIL and ensure compliance with NDIS Practice Standards and internal policies.
  • Continuous Improvement: Identify opportunities for innovation in service delivery and adapt to policy updates within the NDIS framework.
  • Flexible Support: Occasionally support HR functions or other internal operations as needed.

About You

  • Qualification in Social Work, Psychology, Social Sciences, Allied Health, or a related discipline (Essential)
  • NDIS Knowledge with minimum 2 years’ experience in disability case management or NDIS coordination (Essential)
  • Strong understanding of NDIS Practice Standards, risk management frameworks, and the National Disability Insurance Scheme
  • Skilled in person-centred care planning, case documentation, and stakeholder communication
  • Experienced in managing complex behaviours and psychosocial disabilities
  • Demonstrated leadership qualities and the ability to guide team members through performance, training, and crisis scenarios
  • High attention to detail and strong administrative and digital record-keeping skills
  • Excellent written and verbal communication skills
