With a strong foundation in trauma-informed practice and Out-of-Home Care (OOHC), we’re expanding our services to support individuals living with disability through the National Disability Insurance Scheme (NDIS).

SKILLS AND EXPERIENCES:

The successful applicant/s will require the following Qualifications and Experience:

  • Cert IV or higher in Youth Work, Disability Support, Community Services or related field.
  • Current Working with Children Check (Blue Card)
  • Current NDIS Worker Screening (Yellow Card)
  • Criminal History Check (Police Check) within the last 12 months
  • Unrestricted QLD Driver’s Licence
  • Current First Aid & CPR
  • Experience supporting young people with complex trauma (Highly regarded)
  • Skills in case planning, behaviour support, and report writing (Desirable)
  • Over 2 years Leadership experience in a similar role within the Disability sector (Desirable)

As a Service Delivery Manager with Strive Community Care, you’ll step into a strategic position with real impact—shaping the quality of support services delivered to individuals living with disability, while mentoring and empowering your team to thrive.
This Permanent Part-Time opportunity (20 hours per week) offers you the stability to grow with the organisation, with a view to transition to Full-Time hours for the right person. It’s an ideal role if you’re ready to step into leadership—or return to it—with support, balance, and purpose.
You’ll lead from the front, building a positive, high-performance culture, managing support teams, and ensuring that services are not only delivered efficiently, but also with empathy, dignity, and excellence. This role blends people leadership, client relationship management, operational oversight, and continuous service improvement—giving you the chance to grow your skillset and lead change.

SOME OF THE KEY RESPONSIBILITIES INCLUDE:

  • Lead & empower your team: Support and supervise Disability Support Workers and Senior Support Workers, offering feedback, coaching and development to help them excel.
  • Drive service excellence: Oversee and continuously improve support delivery, ensuring services meet NDIS standards, individual client goals, and internal quality benchmarks.
  • Champion client success: Engage with participants and their families to ensure their voice is heard and their goals are front and centre in every support plan.
  • Collaborate & build partnerships: Work closely with support coordinators, plan managers, and local networks to ensure joined-up service delivery.
  • Monitor outcomes & manage risk: Stay across incident reporting, service documentation, audits, and compliance checks to ensure high standards are met.
  • Keep things moving: Manage rosters, resources, budgets and stakeholder updates—balancing the day-to-day while keeping an eye on long-term growth.
