We are seeking a passionate and proactive Full Time NDIS Social Worker to assist clients in navigating their NDIS journey, ensuring they have access to necessary supports and resources.

QUALIFICATIONS AND SKILLS:

  • Bachelor’s degree in Social Work, Disability Services, Community Services, or a related field.
  • Proven 3 year+ experience in the disability sector, particularly with client management and NDIS applications.
  • Strong understanding of NDIS processes, policies, and legislation.
  • Exceptional interpersonal skills with the ability to build trust and maintain supportive relationships.
  • Ability to work independently and as part of a small, supportive team.
  • High level of organisational skills, capable of managing diverse tasks and responsibilities.
  • Demonstrated advocacy skills and a commitment to upholding participant rights.
  • Proficiency with relevant software for documentation and reporting.
  • Valid driver’s licence and access to a vehicle
  • Experience using CRM or care management systems (preferred).
Responsibilities
  • Assist clients with NDIS applications, reviews, and any required documentation for funding.
  • Work with clients to understand their personal goals, needs, and aspirations, ensuring a tailored approach to support.
  • Develop and implement client-centred support plans that foster choice and control in each participant’s journey.
  • Manage the intake process for new clients, including collecting essential documentation and conducting risk assessments.
  • Advocate for participants, ensuring their rights are protected and voices are heard within the NDIS system.
  • Facilitate access to suitable services by identifying, connecting, and coordinating with appropriate service providers.
  • Provide ongoing client management, monitoring progress towards goals, and adjusting support plans as needed.
  • Maintain accurate and up-to-date documentation, reports, and records in line with NDIS guidelines.
  • Collaborate with allied health professionals, community organisations, and government agencies to create a supportive environment for participants.
  • Educate participants, families, and support networks on the NDIS process and available services, empowering them to navigate the system confidently.
  • Facilitate community engagement programs promoting skill development, independence and social connection.
  • Actively promote Health U Australia’s services, brand, and values through professional engagement, networking, and community outreach.
