Wekare is seeking an enthusiastic individual to join our Booragoon team as an NDIS Support Coordinator for 2 days.
NDIS Support Coordination duties:

Main duties and responsibilities include:

  • This primary responsibility of this role is supporting people with a disability to coordinate and implement their NDIS plan. This role is responsible for building a rapport with participants to understand their situation and coordinate the delivery of a range of services ensuring outcomes are met according to each plan.
  • Support Coordination is a role specific to the NDIS that aims to support people to optimise and implement their NDIS Plan and the funding available. It may involve connecting people to mainstream services. It includes supporting people to resolve points of crisis that may occur.
  • Support Coordinators are key to achieving WDSG’s mission and vision and delivering quality NDIS services, by genuinely working alongside people to achieve their goals and aspirations

DUTIES AND RESPONSIBILITIES (Specific)

As a Support Coordinator you will need to demonstrate and maintain the following attributes:

  • Highly developed and evolving skills in NDIS Support Coordination.
  • Understand the role of the mainstream service system
  • Foster a high-level understanding of mental health issues and psychological disabilities.
  • A desire to acquire broad knowledge and or training in areas of social work, occupational therapy, psychology, and related disciplines.
  • Demonstrate and exceptionally high level of integrity, transparency, accountabilit,y and professionalism, especially in relation to billable hours to participants.
  • Consistently demonstrate the ability to remain calm, maintaining a positive and professional outlook while maintaining constructive relationships with all stakeholders.
  • A commitment to maintaining a thorough understanding of NDIS legislation and best practice.
  • Comprehension of the NDIS Price Guide and budget and flexibility.
  • An ability to consistently and reliably complete administrative duties including documentation to a high standard.
  • An ability to work autonomously, efficiently, and effectively to manage time and self-regulate on a casual schedule, while increasing capacity over time.
  • Have a willingness to occasionally provide feedback and advice regarding WeKare Disability Support Group’s, Support Coordination Systems, tools, and processes, to help drive continuous improvement within the organisation.
  • Maintain an insured, roadworthy vehicle and an Australian driver’s licence as an essential requirement of the role.
  • Work with the participant to develop reports on the progress
  • towards goals which they need to provide to the NDIA. Reporting is performed as standard or subject to individual requirements. If no reporting requirements are specified by Plan Management the Support Coordinator must contact the NDIS. If the NDIS does not outline specific requirements then the Support Coordinator must default to Standard reporting: 8-week progress report/9-month plan review report (to be completed at a minimum 3 months before plan expiry).
  • Demonstrate a firm commitment to confidentiality and ensuring the security of participant documentation and information at all times.
  • Maintaining a clean criminal record as an essential requirement of the role.
  • Undertake additional, reasonable responsibilities as directed by WeKare Disability Support Group.
Responsibilities

KEY RESPONSIBILITIES / EXPECTATIONS & WORK PRACTICES

  • Contact new participants promptly as allocated by your team Leader (ideally within 2 days) of the receipt of Request for Service (RFS) from Local Area Coordinator or planner. Meet and greets should occur within 5 days of receipt of RFS.
  • Develop, document, and update a care plan for each participant in conference with participants, plan managers, specialists and other stakeholders.
  • Liaise with and engage NDIS participants to deliver tailored support based on person centred plans, using the strengths-based, trauma-informed approach.
  • Implement the NDIS plan, including choosing preferred options or providers and overtime.
  • Use the NDIS Myplace Portal.
  • Assess all options for mainstream, community, informal and provider supports.
  • Maintain a rapport and develop high quality professional relationships with participants, carers, family and friends,
  • involving them in the implementation of packages and where appropriate, advocate for the participant as needed or requested.
  • Provide high quality services to clients and keep accurate and complete records of progress and outcomes, in accordance with legislative and organisational requirements.
  • Negotiate services and prices, develop Service Agreements, and create service bookings with preferred providers. Make changes to provider Service Agreements and assist participants to understand their responsibilities.
  • Increase your options to link with mainstream or community services, including health, housing and or transport.
  • Provide advice on housing options and life transitions.
  • Notify the Director and your Team leader immediately of any worker or client-related issues or incidents that occur.
  • Maintain planning and reporting documents in line with standard organisational policies and procedures supporting the development of a new service.
  • Represent and promote both the organisational program and the NDIS program on relevant committees, advisory groups, and other events as required.
  • Undertake specialist activities, including assisting with Plan Reassessments by assessing progress toward goals, referring to new or adjusted goals, and considering value for money. Assess whether the participant has achieved their goals, obtained value for money and further troubleshoot any problems in plan implementation.
  • Negotiate and connect participants to appropriate support and services.
  • Use initiative to identify or predict issues that potentially may arise, and assess/resolve Issues with sound problem-solving skills
    Additional responsibilities for Support Coordination - recovery coaching

KEY RESPONSIBILITIES

  • Collaborate with NDIS participants to create individualized recovery plans that reflect their personal goals and aspirations.
  • Provide mentoring, coaching, and emotional support to help participants enhance their recovery journey.
  • Assist participants in building skills for everyday life, decision-making, and self-advocacy.
  • Coordinate with other service providers, support networks, and stakeholders to ensure a holistic approach to care.
  • Maintain accurate documentation and progress reports in line with NDIS requirements.
  • Stay informed about NDIS guidelines and relevant best practices.
