  • High-level organisational and time management skills
  • Proficiency in Microsoft Office Suite and case management systems
  • Current NDIS Worker Screening Check, Working with Children Check, Police Check, and Driver’s Licence with access to a reliable vehicle

Responsibilities

ABOUT THE ROLE

We are seeking a committed and professional Support Coordinator to join our Care and Support Services team on a part-time basis (4 days per week). This role is participant-focused, with no client sourcing required.

The Support Coordinator plays a vital role in:

  • Assisting NDIS participants to understand and implement their plans effectively
  • Building and maintaining strong, trust-based relationships with participants and their families
  • Coordinating and linking participants with appropriate service providers, community supports, and mainstream services
  • Supporting participants to exercise choice and control, fostering independence and confidence
  • Monitoring funding utilisation and ensuring compliance with NDIS requirements
  • Preparing documentation and reports to support NDIS plan reviews
  • Advocating for participants to ensure their goals, preferences, and needs are met
