SKILLS & QUALIFICATIONS:

  • Experience in NDIS support coordination, case management, or disability/community services
  • Strong knowledge of the NDIS framework, pricing arrangements, and compliance requirements
  • Excellent communication, advocacy, and negotiation skills
  • Ability to manage complex client needs and crisis situations effectively
  • Strong organisational and time management skills with attention to detail
  • Proficiency in client management systems and Microsoft Office Suite
  • Relevant tertiary qualifications (e.g., Social Work, Disability, Community Services, or related field) preferred
  • Current driver’s licence and Working With Children Check / NDIS Worker Screening Check
Responsibilities

POSITION PURPOSE:

The NDIS Support Coordinator is responsible for assisting participants to understand and implement their NDIS plans, connecting them with appropriate services, and building their capacity to manage their own supports. This role ensures participants receive the right mix of formal, mainstream, and community supports to achieve their goals while meeting NDIS compliance requirements.

KEY RESPONSIBILITIES:

  • Support participants to understand their NDIS plan and funding options
  • Develop, implement, and monitor individual support coordination plans
  • Connect participants with NDIS providers, mainstream services, and community supports
  • Build participants’ capacity to exercise choice and control over their supports
  • Coordinate service delivery and resolve service gaps or issues promptly
  • Monitor participant outcomes, progress, and satisfaction with supports
  • Provide regular reports to the NDIA and ensure compliance with NDIS guidelines
  • Maintain accurate and up-to-date client records, case notes, and reporting documentation
  • Assist participants with plan reviews and prepare supporting evidence as required
  • Advocate on behalf of participants to ensure their rights, needs, and preferences are met
  • Collaborate with participants, families, service providers, and other stakeholders to deliver holistic support
