ABOUT YOU

We are looking for a sociable and organised Support Coordinator to join the Maple team. The role involves supporting participants in understanding and implementing funded support in the NDIS plan and linking them to the appropriate services. You will be responsible for establishing and maintaining a positive relationship with the participant and stakeholders.

QUALIFICATIONS:

  • Minimum 1 year of Support Coordination experience
  • Understand NDIS legislation, price guide and rules
  • Ability to work independently and part of a team
  • Ability to maintain a high degree of confidentiality and manage sensitive information
  • Not essential, however highly regarded: relevant territory qualification
  • High level of interpersonal skills, including the ability to develop and maintain relationships
  • Proactive
  • Ability to work under pressure
  • Excellent written and verbal communication skills
  • Intermediate in Microsoft Office
  • Current driver’s license.

Experience:

Responsibilities
  • Implement NDIS plan including to assist in choosing preferred options;
  • Assess all options for community, mainstream, informal and provider supports;
  • Liaise with participants and stakeholders;
  • Maintain rapport and build high quality professional relationships with participants and stakeholders;
  • Respond to emails and administrative work in a professional manner;
  • Empower participants to understand, implement and review their NDIS plans;
  • Assess and obtain relevant NDIS documentation with participants during intake and in the lead up to plan reviews;
  • Monitor expenditure and support participants to remain informed about their expenses;
  • Request updates and specific reports by professional and health personnel as required in support of NDIS submissions;
  • Use initiative to identify or predict an issue that may arise, assess and find resolutions;
  • Actively maintain professional knowledge in the areas of disability, working within the NDIS guidelines.
