What’s the role? …… As a Neighbourhood Coordinator, you will play a key role in Lewisham’s Neighbourhood Model, promoting integration between health, social care, and community services. You will coordinate multi-disciplinary team meetings, build strong relationships with GP practices and partners, and help shape care pathways for citizens with complex needs. What’s in it for you? You’ll need excellent organisational skills to manage multi-disciplinary meetings and priorities effectively. A good understanding of Adult Social Care and community health services is essential, along with confidence in using digital tools and data. You should be proactive, adaptable, and able to work both independently and as part of a team. Above all, you’ll share our commitment to equality, inclusion, and delivering the best outcomes for Lewisham’s citizens. Location The job is primarily based in Laurence House, Catford, SE6 4RU, but we operate a hybrid approach where you will be required to work both in the office and from home on a regular basis.
