NEUROANAESTHESIA AND NEUROCRITICAL CARE FELLOWSHIP
We are seeking an exceptional individual to join the Department of Anesthesia and Intensive Care at Beaumont Hospital for a 1 year Neuroanaesthesia and Neurocritical Care Fellowship. This individual will learn and gain clinical experience in all aspects of Neuroanaesthesia and Neurocritical Care.
The Neuroanaesthesia and Neurocritical Care Fellowship is recognised by the College of Anaesthetists of Ireland.
Beaumont hospital is the Irish national neurosugical centre.The successful candidate will spend 4 months in total in neurocritical care, 2 months in each 6 month period.
The succesful candidate can expect to become expert in management of the brain injured patient. The fellow will learn patient management techniques such as management of brain haemorrhage, transfer of the critically ill patient, percutaneous tracheostomy, diagnosis of brain death and care of the organ donor patient.
The fellow will be exposed to all types of invasive monitoring and be expected to develop a capability to understand and use common Neuro ICU diagnostic and monitoring modalities including EEG, intracranial pressure via various types of monitors, magnetic resonance imaging, computed tomography, evoked potentials, electomylogram, cerebral angiography, transthoracic and transoesophageal echocardiography.
The fellow will be expected to take a leadership role in the intensive care unit and become actively involved in our multidisciplinary team.
Eight months will be spent in the operating theatre dedicated to the practice of neuroanaesthesia. The fellow will be expected to carry out preoperative asssessment of the neurosurgical patient incuding optimisation and risk stratification. The fellow will be expected to have developed a comprehensive preoperative, intraoperative and postoperative care plan for all patients requiring neurosugery.
The fellow will be exposed to all types of neurosurgery and become expert in the provision of neuroanaesthesia for vascular neurosurgery including aneurysm surgery, excision of spinal and cranial arteriovenous malformation and extracranial-intracranial bypass, epilepsy surgery including awake craniotomy, cortical electrode seizure foci mapping and lobectomy in adults and children aged over 6 years, tumour resection both benign and malignant of spinal cord and brain. Craniotomy can occur in a variety of positions including prone, park bench, lateral and sitting. Complex spine surgery including tumour resection, spinal fusion and decompression at all spine levels. Surgery both on brain and spine requiring intraoperative electroencephalographic or somatosensory and motor evoked potential monitoring. Fibreoptic intubation is practiced widely in our department.
The fellow will spend a period of time in our interventional radiology department providing anaesthesia for coiling of aneurysms and embolistation of arteriovenous malformations, meningiomas and tumours of spine and brain. The fellow will also provide anaesthesia for children requiring mri for brain and spinal cord. A 2-week period will be spent in pain management learning the diagnosis and management of acute and chronic pain.
EDUCATION
The fellow will be expected to participate in the education of trainee doctors and other allied healthcare professionals in the practice of neuroanaesthesia and neurocritical care. This will involve tutorials, journal review, and presentation at depatmental meetings.
Responsibilities:
Clinical duties include providing anesthesia care in the operating theatre, intensive Care Unit and pre-operative assessment clinic. Fellows will be expected to participate in an on-call rota for theatre and ICU.

Successful completion of a specialist training program in anaesthesia and are eligible for specialist registration in anaesthesia or eligible for certificate of completion of specialist training is desirable.
