POSITION LOCATED IN NEW BEDFORD   JOB SUMMARY: Under the supervision of their assigned supervisor, the Caseworker is responsible for ensuring  that all clients receive professional, safe care in a well maintained and secure environment; limit setting, behavior management, family work, the teaching of daily living skills, facilitation of medical/dental appointment and follow-up, school tracking, and facilitate psycho-educational groups.  The Caseworker acts as a liaison with the client’s family, Department of Children & Families (DCF) Social Worker, school placements, and medical/dental practices to ensure that families and providers understand clients’ needs and that basic needs are being met.   JOB FUNCTION: Program/Department Responsibility: * Advocate for clients in their home, school, and the community according to the individual treatment plan on a daily basis. * Identify community providers and medical and dental needs of clients following intake. * Schedule appointments as needed with medical/dental providers. * Seek out medication orders/PRN protocols forms/documentation of immunizations and most recent routine appointments for each client. * Facilitate medical/dental/eye appointments for clients as needed to best maintain optimal physical/medical/dental health of all clients * Share information with clinical team, administration, nurse and supervisors following appointments/phone consults through thorough and efficient documentation. * Complete daily school tracking requirements * Participate in weekly client review with administrative team * Coordinate urine toxicology screenings for youth when ordered by provider * Present in a professional manner and attire while representing the OCY DCF Program including personal vehicle, which will be used in fulfilling job responsibilities. * Complete all paperwork and documentation as needed including but not limited to: Medical Encounter Forms, Med Order Forms, PRN Protocol Forms, Narratives, Electronic Appointment Spreadsheet. School tracking log, Youth Readiness Assessments, etc. * Develop professional relationships with local health care providers as well as employers in order to facilitate collaboration between DCF program and local community. * Work with DCF and parents to empower parents to be active in all aspects of their child’s health and education through open communication and collaboration with parent/guardian when appropriate. * Provide rotating on-call clinical coverage and be available for after-hours emergency situation or intake. * Participate in all EEC and CARF related requirements, meetings and visits. * Contribute to a positive work environment by illustrating pride in job performance, enthusiasm and cooperation with co-workers.   * Facilitate and support PAYA programming for youth. Qualifications * Bachelor’s Degree in a Human Services related field or with 1 – 5 years of applicable experience. * Strong knowledge of Microsoft programs is required. * Excellent reading and writing skills for contract and incident reporting. * Ability to communicate with clients, families, and collaterals in person and by phone * Daily attendance and scheduling flexibility * Ability to assist with the restraining of residents up to approximately 200 lbs. * Must be able to drive, provide a safe driving record and have a valid driver’s license
