The New Patient Navigator serves as the first point of contact for new patients. This role ensures a smooth onboarding process by helping patients understand their care options, scheduling initial appointments, assisting with paperwork, and removing any barriers to care. Key Responsibilities: Collect and verify patient information, insurance details, and health history. Schedule initial appointments and coordinate with office and other departments. Provide patients with clear information about our services, providers, and policies. Educate patients on what to expect as a Community Health Care patient. Act as a liaison between patients, providers, and administrative staff. Identify and address potential barriers to care (e.g., transportation, language, forms). Maintain accurate documentation and patient records. Qualifications: High school diploma or equivalent required; associate or bachelor’s degree preferred. Experience in a healthcare or customer service setting strongly preferred. Excellent communication, interpersonal, and organizational skills. Ability to multitask in a fast-paced environment while maintaining attention to detail. Empathetic, patient-focused, and culturally sensitive. Proficient with computers and electronic health records (EHR) systems.
