Description

Provide specialist assessments and reviews of health care needs for NHS Funded Care and Continuing NHS Health Care patients in liaison with Primary Health Care Teams, Social Services, Independent Sector and Voluntary Sector.
Work with the Team Leader to provide professional nursing advice to care homes (nursing), District Nursing Services and partner organisations on a range of physical and mental health nursing issues.
Work in collaboration with partner organisations and other professionals to ensure patients’ health care needs are met appropriately within the care home and community setting.
Assess and review the patient’s eligibility for NHS Funded Nursing Care in care homes and patients own home as appropriate.
Review the health care needs of patients receiving Continuing NHS Health Care in care homes and community setting.
Make recommendations following Specialist Nursing Assessment.
Refer for a multidisciplinary team assessment those patients’ whose physical and mental health care needs have changed.
The post holder will be expected to undertake Best Interest Assessor training.
Through assessment, identify patients who are at risk and act accordingly / manage any issues related to the findings.
Daily conflicting demands which can result in a change of focus and disruption to working pattern
Report any concerns related to patient care following review or assessment to the team leader/ line manager within the Long Term Care Team. Work in partnership with Social Services, CIW, Adult Protection and Social Services as required.
Undertaking complex and challenging meetings with patients/families/outside agency colleagues/staff
Maintain robust Specialist Nursing Assessments/Reviews and care planning records in line with relevant Health Board and Welsh Government policy.
Participate in the joint monitoring of care homes and provide comprehensive and
detailed reports within the designated timeframe.
Take a lead role in the protection of vulnerable adults process (POVA).
