This position will work closely with the psychiatry inpatient unit and the transition team to provide assistance with clients transitioning from hospital back into the SHA housing partnership community beds. Responsibilities include intensive case management and stabilization, medication management, crisis intervention, counseling, goal planning, establishment of community supports through consultation with community agencies, student mentoring, documentation, and client/family education. Human Resources Exemption: No Experience 3 years experience in a Mental Health setting.
