Position #: V41447 Expected Start Date: November 17, 2025 Union: SUN Facility: Wolseley Memorial Integrated Care Centre City/Town: Wolseley Department: Chronic Resident Unit Type: Full-time regular FTE: 1 Shift Information: Days Hours of Work: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Field Hours: No Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: No Job Description: Reporting to the Facility Manager, the Facility Care Coordinator is responsible for ensuring a high standard of care and service is provided to patients and residents at Wolseley Memorial Integrated Care Center (WMICC). WMICC is comprised of 78 beds in Long Term Care, 12 Bed Acute Care and OPD services. As an experienced practitioner, the facility care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of client care, and support and teaching to the client care teams (including clients and their families) within a collaborative environment. Human Resources Exemption: No Experience Supervisory experience is an asset Minimum of 3 years clinical nursing experience Recent acute and long term care experience required.
