Ensures that the care provided is in accordance with the organization's mission, care philosophy, client care plan, policies, procedures and standards of care. Human Resources Exemption: No Experience 3 yrs experience in Palliative Care (combo of acute and community preferred) Education Bachelor of Science in Nursing (BScN) Licenses Valid Class 5 driver's license Other Education and Training Nursing diploma is acceptable LEAP Certification (Learning Essential Approaches to Palliative and End of Life) Competencies Critical thinking skills Decision making skills Leadership skills Advanced Clinical Assessment skills Conflict resolution skills Knowledge and Abilities Ability to prioritize Ability to communicate effectively both orally and in writing Ability to integrate theory/knowledge/experience to enhance client centered care Ability to provide clinical recommendations to HCP, clients and their caregiver Able to plan/organize/prioritize work w/in a continuously changing environment Ability to competently use computers and software applications Ability to lead best practice Commitment to continued learning and professional development.
