The PHC RN promotes quality health care outcomes working within established standards for case management practice to ensure that patients/clients receive a high standard of primary health care. This will be done in accordance with Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department.   Human Resources Exemption: No   Licenses: * Valid Class 5 driver's license Education: * Bachelor Degree in Nursing, Master level preferred Competencies: * Leadership skills * Critical thinking skills * Communication skills * Public relations skills * Organizational skills Experience: * Minimum 2 years nursing experience in primary care, public/community health * Experience in teaching adults, facilitation skills, and leading group activities * Experience in case management is an asset Knowledge & Abilities: * Ability to take initiative * Demonstrated clinical competency * Knowledge in chronic disease prevention and management * Demonstrated ability to work in a multidisciplinary setting * Ability to adapt to and implement change * Ability to be self-directed  * Advance computer skills, including Microsoft Office (Excel, Word and Access).
