Description The Nurse Care Coordinator (RN) plays a vital role in advancing high-quality, patient-centered care within our Federally Qualified Health Center (FQHC). They will utilize care coordination, case management and resource management with the purpose of improving the patient’s health, reducing healthcare costs and improving the patient’s healthcare experience. They will collaborate with the patient, the patients primary care physician and other care team members including outside entities and community health partners to help the patient achieve their healthcare goals. Their work may encompass a focus on medical, mental health, substance use and social determinants of health.
