Mid-Michigan Recovery Services (MMRS) is looking to hire a Nurse Care Manager who wants to make a difference! With MMRS, you will play a vital role providing health screenings and assessments, health coordination, medical support and wellness services, for medically vulnerable clients, across the agency. This role is ideal for a motivated healthcare provider who thrives in a collaborative, mission-driven setting and seeking growth. Qualifications * Current and unrestricted Nursing License in the State of Michigan LPN can be considered based on scope and supervision structure. * Experience in Behavioral health, substance abuse treatment or community health preferred. * Knowledge in medication assisted treatment, chronic disease management and healthcare systems. * Training or experience in infection prevention and control, wound assessment and wound care * Strong leadership, communication, prioritization and organizational skills. * Strong clinical judgement, communication skills, critical thinking and crisis response skills.
