As a Nurse Care Manager, you will support clients by coordinating medication reconciliation, engaging with medical providers, and helping individuals follow through with medical care and recommendations. Responsibilities: Support enrollees with health education, care coordination, and 1:1 training based on individual needs Offer guidance on medical topics including medications, conditions, and treatment options Lead health and wellness groups covering topics like nutrition, exercise, smoking cessation, and chronic illness prevention Help enrollees access community resources that support their health and wellness goals Work closely with enrollees and team members to increase understanding of medical conditions and overall health management Qualifications: Requires an Associate's degree in Nursing or equivalent Experience working with adults with severe mental illness Valid Massachusetts Drivers License Valid LPN or RN license from the Commonwealth of Massachusetts Schedule: Monday-Friday, 9am-5pm Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase.
